Clare is passionate about medicines safety. She has worked in the National Health Service acute hospital sector for the last 26 years. She is a Consultant Pharmacist in Medicines Safety, and Medicines Safety Officer at the Oxford University Hospitals NHS Foundation Trust in Oxford, England, where she leads a multidisciplinary team ensuring that medicines safety issues are identified and addressed and that learning occurs across the organisation. She has always maintained a strong clinical focus to her work.
Clare has been working within safety since 2002 with a particular interest in high-risk medicines, procurement for safety, and injectable therapy. This led to her PhD studies at Cardiff University investigating injectable medicine preparation errors in clinical areas. She is actively involved with regional and national safety initiatives and is a keen advocate of quality improvement. As a University of Reading Visiting Fellow, she teaches and collaborates on safety and resilience projects. Her growing understanding of complex systems has informed her interest in applying human factors to healthcare safety. In her session, she will share some of the current thinking about patient safety and drivers for doing things differently.
